You can find various forms you will need while being a patient of Dr. Peavy below. These forms are only templates and every case is unique. You will be given a case specific form and have a chance to ask questions and clarify issues before any treatment is rendered.

Anesthesia Request
The form you will need to fill out if you are requesting sedation.
Chaperone Information
If you are recieving anesthesia, you will need to give this form to whomever will be escorting you from our offices.
Crown Lengthening Request
